Emperor Palpatine (Return of the Jedi)

Tetsuo (Akira) - While not a "death" per say however he takes out all of Tokyo with him.

The Film Actors Guild (F.A.G.) members (Team America)

Starscream (Transformers: The Movie)

Gendo Ikari (The End of Evangelion)

Henry Fonda in Once Upon a Time in the West.   Close-up of Fonda on the ground with his harmonica falling out of his mouth and Charles Bronson in the background.

Lo-Pan (Big trouble in little china)
