I don't full recall what it was either I just used waistband of his pants as sort of a "default" position. I could see it being a bandage/wrap or a back brace or something. I've tried Googling the "stereotype" as well on the nipples of Japanese men and just come up lost.  Perhaps it is not for us to know.

Watched it again and the guy was wearing some-king-of band/garment/girdle around his waist.  The camera still oddly focuses on the guy's nipples and the guy still tugs it over his nipples but I'm guessing either seeing the garment or maybe just over-hearing their conversation Lee made the conclusion they were Japanese.  Still very odd.

Yea I re watched it do and his nipple was clearly established as the point of interest.
