Why do all animals have their food intake orifice in their head? Why not the chest, or some other part of the body?
You'd think it would have happened with some animal at some time.

Sea stars turn their stomachs inside out, I think?

I guess I should have specified an animal with a distinct head and limbs. Higher organisms that are not amoeboid or radial.

Proximity of nose to mouth is important (most of what we taste is actually smell, partly going up the back access to the olfactory nerves via the back of the throat's connection to the nasal passages).  Also: the smaller the distance between the two the more likely it is that if one smells something 'good', that is what is more likely to end up in one's mouth, and if one smells something 'bad' next to it, that it doesn't.  If the nose and mouth were far apart, likely it would require activity by something like a hand (or at least still more articulation of the skeleto-muscular system) in order to get the 'tasty'-smelling things into it.

Possibly also at play is that it is 'simpler' to have a tube stretching basically from one end of the body to the other through which food can pass.  Though it still is massively convoluted (small intestines a bit over 20 feet, coiled upon itself), it would be more difficult to line up the appropriate organs to play their respective roles with shorter distances between the input and output holes.

In addition, the short distance of four of the five senses to the brain means that there is less intermediary tissues between them and the brain, so information processing is quicker, less likely damage will occur between the two, and the chance for something going wrong during gestation is also limited accordingly.  (Obviously the sense of touch for most of the body requires the spinal cord, which adds a whole 'nother system that can get messed up... and often does.)

Those are a few I can come up with offhand, I'm sure there's more, but I'm not thinking well (a week of heavy seizure activity [and attendant increase in meds] has left my thinking fuzzy).

Yes it's more that all of that stuff evolved that way interdependent on each of the other elements.

To add to what LucasM said, the eyes are also important, and in many animals whiskers around the mouth.

Nerve impulses don't travel super fast, on very large animals it can take a second for a signal to travel the entire length of the body.  Even with the relatively small size of the human body we still have reflexes controlled by the spinal column, to avoid the delay of a signal going all the way to the brain.

All those inputs that require fast response times evolved and remain close to the brain, so pretty much how ever the rest of the body evolves, eyes, ears, nose, and mouth all remain near where ever the brain is.
